This network of organizations, providers, payors, and local/state government is connected through a shared technology platform, allowing us to coordinate patient care through referrals and connect our clients to the social services they need.
Our partners provide a broad range of services such as housing, employment, food assistance, behavioral health, utilities, childcare support, transportation access and more.
